destra in fase acuta. Il paziente è stato operato in coma con anisocoria
OD > OS dopo alcune ore dall'evento ictale manifestatosi con emiplegia
sinistra. Il recupero è stato totale. È significativa la
particolare conformazione dell'albero arterioso: la cerebrale anteriore
di destra origina dal sifone carotideo di sinistra da cui nasce anche la
cerebrale posteriore omolaterale. L'assoluta indipendenza della carotide
interna dagli altri vasi intracranici, rende l'emisfero cerebrale omolaterale
particolarmente vulnerabile all'evento trombotico. La sua riabilitazione
però può favorire la guarigione.

The patient was operated during coma with anisocoria RE > LE after few hours since the ictal event occurring with left hemiplegia. The recovery has been total. It is significant the particular shape of the arteries: the right anterior cerebral artery derives from the left carotid siphon. From the siphon is originated the homolateral posterior cerebral artery as well. The absolute independence of the internal carotid from the other intracranial vessels makes the homolateral cerebral hemisphere vulnerable to the thrombolic event. On the other hand, its perfusion may facilitate the recovery.
